Our New Patient Forms
During your first visit, you are requested to complete initial forms so that we can better understand your history and meet your needs. You are able to download our forms by clicking on downloadable forms above, printing them out, and bringing the forms with you. You will also be asked for your insurance cards and any referrals that are required by your insurance.
Initial Examination
When the doctor examines you, a thorough foot assessment is conducted and if X-Rays are necessary, they are done on the premises. The doctor can then discuss any findings with you and together you will determine the most appropriate foot care for you. If any further procedures are recommended, the office staff is able to assist in setting up additional office appointments, diagnostic studies, or surgery, as appropriate. Our goal to make all your appointments as informative as possible so if you have any questions or require further information, do not hesitate to ask the doctor or one of our certified podiatric medical assistants, surgical technologists, medical assistants, receptionists, or insurance billing specialists.

When to Arrive
Arrive 15 minutes before your scheduled appointment time to complete any forms. New patients may require as much as 60 minutes for their initial evaluation depending on their condition.
